Data submission and claims for payment
The monthly data submission claim form (cataract, IOP/GAT and OHT schemes combined) should be completed and emailed to leedswestccg.opticians@nhs.net
A monthly invoice for the corresponding payment claim can be submitted via post (to the address below) or electronically via a Tradeshift account.
