PES Practice & Practitioner Documentation Review

PES Operations & Governance Team have been reviewing practice documentation. Please upload any outstanding documentation as a matter of priority. These include the following list:

Practice Documents Required:

• DSPT (Data Security and Protection Toolkit)
• GOS Contract
• NHS Contact Checklist
• Annual Declaration

Practitioner Documents Required:

• DBS Certificate
• Safeguarding level 2 Children
• Safeguarding level 2 Adults
• Annual Declaration

Failure to comply may lead to practices being restricted access to OPERA and therefore to provide services.
