Routine Referrals
You may only do this using your own PRACTICE nhs.net account to keep patient details secure; photographs of referral letters are not allowed as this breaches NHS confidentiality and security guidelines in your NHS contract.
Email referrals to: licb.referrals.lincolnshireeach@nhs.net
Referrals are then triaged by a panel of accredited optometrists either into the Community Optometrist Triage And Treat Service or secondary care.
A copy of the referral should be sent to the patients GP for information only.

If an appointment becomes inconvenient or the patient no longer wishes to attend- please ask the patient to make contact with ULHT (United Lincoln Hospital Trust) using the numbers below:

Please send urgent referrals (wet AMD type referrals including anything that you would have previously faxed directly to the hospital should now be sent as an email to:

LVA assessment referrals should be sent to the following email address

Please make it clear that it is for LVA assessment and which location is preferred by the patient: Grantham, Sleaford, Ruskington, or Lincoln.
