BSW ICB Routine Referrals
Most of the routine referrals within the Wiltshire, BANES and Swindon areas should be sent via the triage service to bswicb.rsseyes1@nhs.net. The team of Optometrists will then arrange for the appropriate onward referral.
Most practices will have their own referral templates to use for this. If not please use the standard GOS18 referral form.
Note; for glaucoma referrals, the referral centre requests- a brief disc description, IOP, visual fields and an open or closed Van Herrick assessment of the anterior chamber angle to be included in the referral.
