Optometrists can now refer Chalazions using the PLCV criteria

12 May 2025

Chalazion and Meibomian cyst referrals now live via Eyecare eRS for optoms

Optometrists are now able to refer patients directly for Chalazion and Meibomian cysts to University Hospital Derby and Burton and Chesterfield Royal Hospital. GPs will no longer process Chalazion and Meibomian cyst referrals as the Optometrist will be able to check against the PLCV Criteria themselves (similar to current Cataract referral process). Please note – Px’s that do NOT meet the PLCV Criteria are not eligible for referral.

The direct referral will be via eERS on Cinapsis where available. (RDH already online, CRH coming soon).

The Eyecare eRS system will remind Optometrists of the criteria the patient must meet to be referred into secondary care for Chalazion and Meibomian cysts and ask the Optometrists to confirm the criteria are met before submitting the referral.

A reminder of the criteria is below:

Derby and Derbyshire ICB, in line with its principles for procedures of limited clinical value has deemed the Incision and Curettage OR Intra-Lesion Steroid Injection of a Meibomian Cyst should not be routinely commissioned unless TWO OR MORE of the following criteria have been met:

• Has been present for more than six months

• Has been managed conservatively with warm compresses, lid cleaning and massage for at least FOUR weeks

• Vision is significantly impaired

• Lid closure is affected, thereby compromising eye protection

• Where it is a source of infection, creating an abscess

• It has been a source of infection, requiring medical attention on two or more episodes in the last six months.

This change will streamline the referral process for the optometrist and patient.
