Glaucoma Enhanced Referral Service (GERS)

Please see below for the pathway for glaucoma patients across Northumberland, North Tyneside and Newcastle Gateshead.

Please remember that patients who have a sight test at your practice and who are identified as having raised IOP and/or glaucomatous visual field loss with no other indication of glaucoma should be seen within GRR.

Patients who are referred to your practice or who have other indications of glaucoma should be seen within GERS.

If a patient requires a referral to NuTH following their GERS assessment. Please remember to attach their OCT and visual field images to the referral. This can be done during the red tab referral process on OPERA.

We are aware of a number of referrals that have been actioned and sent without OCT and other images. OCT and visual field assessment are a mandatory part of the service and should be attached to all referrals. We will be monitoring this on a monthly basis moving forward.
