REQUEST FOR PRACTICES TO JOIN RNIB PILOT

Dear Constituents,
Pilot call out to primary care Dorset
Please find above, a Pilot Call Out from Stephen Weymouth of the RNIB who is looking for interested practices to join, with a view to feeding the information gained back to the ICB for a possible new service in the future.
Practices would simply have to give a leaflet or a card to patients following any Ophthalmology referral apart from minor eye conditions. This includes NHS or private referrals, to UHD, CHEC or Spa Medica/other ISP.
The leaflet will guide the patient to trackable links where they can access more information about their eye condition, one being the RNIB support page and the other to sight condition pages. The leaflet will also include contact information for the Eye Care Liaison Officer. This will give the RNIB valuable information about how many patients looked for further information following their referral. It will also help to reduce the number of no shows at UHD and the number of patient requests at practices for further information following their referral.
